Physicians Order
Choosing a skilled nursing facility, whether for yourself or a loved one, can indeed be a daunting task. At Baywind Village, we fully comprehend the challenges this decision entails. Our dedicated admissions staff at Baywind Village is wholeheartedly focused on offering you the essential information and support required not only during the admissions process but also as you continue your journey with us.
Every patient is required to have a physician’s order to be considered for admission, regardless if they are currently in the hospital or not. Our Medical Director can evaluate your loved one, and if need is established, can write the order.
Patient Information
The following information will assist us in understanding your individual needs and complete the admissions process:
- History & Physical (H&P) – provided by your doctor or hospital
- Insurance Cards, copies will do (Medicare, Medicaid, BC/BS, AARP, etc)
- Social Security Card
- Copies of Advanced Directives (living will, medical power of attorney, DNR, etc.)
- Name, Address, Phone Number of Responsible Party
If your loved one is leaving the hospital, most of this information will be supplied by the hospital’s Discharge Planner.
Many of our residents have been referred by hospital Discharge Planners. They are an invaluable resource and we work closely with them, but the ultimate choice in facility lies with the family.
