At Baywind Village we realize that choosing a skilled nursing facility for you or your loved one can be a challenge. We are dedicated to making the process as simple as possible and want to assist our families in making the transition seamless. It is important to understand the admissions process in order to make an informed decision about the best facility for your needs. Our admissions staff at Baywind Village Village is dedicated to providing you the information and support you need during the admissions process and beyond.
Physicians Order
Every patient is required to have a physician’s order to be considered for admission, regardless if they are currently in the hospital or not. Our Medical Director can evaluate your loved one, and if need is established, can write the order.
Patient Information
The following information will assist us in understanding your individual needs and complete the admissions process:
- History & Physical (H&P) – provided by your doctor or hospital
- Insurance Cards, copies will do (Medicare, Medicaid, BC/BS, AARP, etc)
- Social Security Card
- Copies of Advanced Directives (living will, medical power of attorney, DNR, etc.)
- Name, Address, Phone Number of Responsible Party
If your loved one is leaving the hospital, most of this information will be supplied by the hospital’s Discharge Planner.
Many of our residents have been referred by hospital Discharge Planners. They are an invaluable resource and we work closely with them, but the ultimate choice in facility lies with the family.
